Your Visa

There are five easy steps to applying for a visa:

1. Fill out the forms;
2. Make appointment for interview;
3. Pay application fee and purchase courier bag;
4. Obtain passport photo and gather supporting documents;
5. Attend interview.

Final Checklist

• Before you attend your consulate interview please review this checklist to ensure that you have everything required.
• DS-156 Form - Completed online, printed and signed 
• DS-157 Form - Males Only, aged 16 to 45 
• Valid passport
• Visa application fee receipt – payable at any NZ Post Shop prior to your interview
• Pre-paid, self-addressed "track and trace" courier bag
• One passport sized photo
• Supporting documents 
• If applicable, your Criminal History Report. You need this only if you check “yes” in block 38 of the DS-156. 
• DS-158 Form 
• SEVIS fee receipt 
• DS-2019 form 
• Acceptance letter from your visa sponsor 
• Copy of your job letter or placement letter in the U.S., if applicable.

If your particular program requires that you are enrolled full-time at a tertiary institution, a letter from your institution confirming your full-time student status or a copy of your transcripts (unofficial web version is fine.)
